What Happens During a Diabetic Eye Exam?

A diabetic eye exam is a specialized comprehensive examination focused on detecting and monitoring the effects of diabetes on the eyes. This includes diabetic retinopathy, diabetic macular edema, and increased risk of cataracts and glaucoma.

Who Needs This Service

All people with Type 1 or Type 2 diabetes. Type 1 diabetics should begin annual exams within 5 years of diagnosis. Type 2 diabetics should begin annual exams at the time of diagnosis. Pregnant women with pre-existing diabetes should be examined in the first trimester. More frequent exams are recommended when retinopathy is detected.

What to Expect

A comprehensive dilated exam with specific focus on: macular evaluation for diabetic macular edema (fluid swelling), peripheral retina evaluation for hemorrhages, exudates, and neovascularization, optic nerve assessment, and intraocular pressure. We use ultra-widefield retinal imaging to capture a broader view of the peripheral retina. OCT imaging provides cross-sectional views of the macula to detect subtle fluid accumulation.

My blood sugar is well controlled — do I still need yearly exams?

Yes. Even with good control, complications can develop over time. Annual exams allow early detection when treatment is most effective.

Will the eye exam affect my blood sugar?

A dilated eye exam itself does not affect blood sugar. If you are coming early in the morning, eat your normal breakfast and take your medications as usual.

I was just diagnosed with diabetes. When should I schedule an eye exam?

As soon as possible after your Type 2 diagnosis, or within 5 years of a Type 1 diagnosis. This establishes a baseline to compare future exams against.

What happens if diabetic retinopathy is found?

The severity determines management. Mild to moderate cases are monitored more frequently. Significant retinopathy is referred to a retinal specialist for laser treatment or anti-VEGF injections.
